The Prophet Sallalahu alaihi wa sallam said to gift each other with gifts because it increases love, and to accept it even if it were trotters of a sheep (meaning accept it even if you don't like it)

 Also, since birthdays aren't allowed in Islam, there shouldn't be a special occasion why you give your spouse a gift. It doesn't have to be a materialistic gift. It could be a thoughtful card or a box of chocolates or a special dinner. 

I feel gifts aren't mandatory and you shouldn't be expecting for a gift, but if you can surprise your spouse with something they really need or wanted for a long time, it'd be kind of you to do that just to make them happy and increase the love between each other. This is just my take on it.
